ABSTRACT

El objetivo de este estudio fue determinar la capacidad de agencia de autocuidado en245 pacientes hipertensos que asistieron a una institución de segundo nivel de atenciónen el 2010, mediante el desarrollo de un estudio descriptivo de tipo transversalcon la aplicación del instrumento Evaluación de la capacidad de agencia de autocuidadoen el paciente hipertenso. Se encontró que el 81,22 % (n = 199) y el 8,16 % (n = 20)de los pacientes presentaron una capacidad de agencia de autocuidado media y bajarespectivamente, y tan solo el 10,61 % (n = 26) de los pacientes presentó una capacidadde agencia de autocuidado alta. Ello indicaría que aunque el paciente con hipertensiónarterial posee algunas habilidades que le permiten discernir acerca de los factores quedebe controlar para promover su salud y su cuidado, son insuficientes para lograr unaadecuada adherencia al tratamiento, ya que para alcanzar un compromiso y participaciónen este, es necesario que existan conductas permanentes de autocuidado quefaciliten el autoconocimiento, el empoderamiento del individuo y el desarrollo de lacapacidad de tomar decisiones sobre su salud. The objective of this study was to determine the self-care ability on 245 hypertensivepatients who attended to a secondary level institution in 2010. This was done by developinga descriptive cross-sectional study with the application of the “Assessmentself-care ability instrument in hypertensive patients” (Evaluación de la capacidad deagencia de autocuidado en el paciente hipertenso). We found that 81.22% (n = 199) and8.16% (n = 20) of the patients presented a medium and low Self-care ability respectively,and only the 10.61% (n = 26) of the patients presented a high self-care ability.This would indicate that, although the patient with hypertension has some abilitiesthat let him/her discern about the factors that he/she should control to promote hishealth and health care, they are insufficient to achieve an adequate adherence to thetreatment. In order to have their commitment and participation, it is necessary to havepermanent self care behaviors to facilitate the self-awareness, the empowerment of theindividual and the development of the capacity to make decisions about their health.The patient's perception of his health is critical to the development of the self careabilities, so it is essential that the nurse intervene in accordance with the individual'scapacities to self-care, by promoting educational support to allow the patient to modifyhis behavior, acquiring knowledge and skill...


O objetivo deste estudo foi determinar a capacidade de agencia de autocuidado em245 pacientes hipertensos que compareceram a uma instituição de segundo nível deatenção em 2010, mediante o desenvolvimento de um estudo descritivo de tipo transversalcom a aplicação do instrumento Avaliação da capacidade de agencia de autocuidadono paciente hipertenso. Encontrou-se que 81,22 % (n = 199) e 8,16 % (n = 20)dos pacientes apresentaram uma capacidade de agencia de autocuidado média e baixarespectivamente, e somente 10,61 % (n = 26) dos pacientes apresentou uma capacidadede agencia de autocuidado alta. Isso indicaria que, o paciente com hipertensãoarterial possui algumas habilidades que lhe permitem discernir sobre os fatores quedeve controlar para promover sua saúde e seu cuidado, são insuficientes para conseguiruma adequada aderência ao tratamento, já que para alcançar um compromisso eparticipação neste, é preciso que existam condutas permanentes de autocuidado quefacilitem o autoconhecimento, o empoderamento do indivíduo e o desenvolvimento dacapacidade de tomar decisões sobre sua saúde. ABSTRACT

Objective To test the reliability and validity of international physical activity questionnaire (IPAQ) in patients with hypertension.Methods It used international physical activity questionnaire and PA diary to evaluate the physical activity of patients with hypertension.Results The two re-test coefficients of IPAQ were related in hypertensive patients with different blood pressure level and physical activity.The energy consuming of different level of physical activity and total physical activity was correlated to each dimension of IPAQ.The first Spearman coefficient was 0.27~0.61,0.28~0.58 for the second time.Conclusions IPAQ had better reliability and validity,and could be used in hypertensive patients.

ABSTRACT

Objective To explore the therapy effect of candesartan plus CSDP on elderly hypertensive patients with renal dysfunction.Methods Eighty elderly patients with essential hypertension and renal dysfunction were selected as our subjects.All subjects were randomly divided into candesartan group and candesartan + CSDP group,every group for 40 subjects.Patients in candesartan group were treated with 4 mg candesartan once per day for 6 months.Patients in candesartan + CSDP group were given CSDP 10 capsules three times / day beside 4 mg candesartan,once per day for 6 months.The levels of Cr,BUN,Ccr changes were measured.Results Two therapy strategies can decrease blood pressure and the decrease rate of blood pressure in candesartan + CSDP group was higher than that of candesartan group.The level of Cr,BUN in patients with candesartan + CSDP group after treatment performing were (104.6 ± 6.5) μmol/L,(6.21 ± 4.80) mmol/L,lower than that in before treatment (196.8 ± 2.6) μ mol/L,(9.84 ± 3.20) mmol/L.And the Ccr increased from (58.1 ± 4.9)ml/min to (117.2 ± 6.4)ml/min.The differences were significant (t =2.213,2.302,2.136,P < 0.05).However the differences between before and after treatment with only candesartan treat weren't significant (P < 0.05).Conclusion Candesartan + CSDP might show a synergistic effect compared with candesartan in term of curing elderly hypertensive patients with renal dysfunction.

ABSTRACT

<b>Objective:</b> The present study examined the status of the provision of drug information to hypertensive patients at community-based pharmacies by evaluating patients' satisfaction, understanding, and wishes.<br><b>Design:</b> A cross-sectional study was conducted by surveying patients using a self-administered questionnaire.<br><b>Methods:</b> Three items were evaluated: 1) the level of patient satisfaction with provision of drug information; 2) the level of patient understanding of information provided on antihypertensive drugs; and, 3) patient wishes regarding provision of drug information.<br><b>Results:</b> The level of satisfaction with provision of drug information was high (86.6%). Although patients were found to understand the precautions for use of antihypertensive drugs, their understanding of drug efficacy was insufficient. Also, patients wished to receive more explanation from pharmacists regarding the efficacy of drugs in addition to explanations of associated adverse reactions or precautions for use of the drugs.<br><b>Conclusions:</b> Hypertensive patients were highly satisfied with the provision of drug information at community-based pharmacies, but did not show sufficient understanding of the efficacy of antihypertensive drugs, an issue to be addressed in the future by pharmacists in their instructions to patients on the use of drugs.

ABSTRACT

PURPOSE: The purpose of this study was to identify and profile distinct subgroups of patients with hypertension based on knowledge, attitudes, and behavior regarding their medication and health lifestyle. METHOD: A descriptive-exploratory research design was employed. Two hundred and twenty-three patients with hypertension using W Public Health Center were randomly recruited on the basis of being over 30 of age. Upon the receipt of their written consents, direct interview with a structured questionnaire were conducted by a public health nurse. Descriptive statistics and chi2-test were utilized. RESULTS: Three subgroups were identified. Group I members had a low score on all three factors including knowledge, attitudes and behavior related to medication and health lifestyle. Group II members had a low score on one or two of the factors. Group III members had a high score on all three factors. The three subgroups were significantly associated with education level, economic status, non-smoking and non-drinking. CONCLUSIONS: Further research should be conducted to validate these findings and test tailored nursing intervention for patient compliance.

ABSTRACT

BACKGROUND: This study was designed to determine the efficacy of a combined use of esmolol and nicardipine for blunting hypertension and tachycardia after tracheal intubation in hypertensive patients. METHODS: Forty-five hypertensive patients were randomly divided into three group: group E (esmolol 0.5 mg/kg, n = 15), group N (nicardipine 30microgram/kg, n = 15), group EN (esmolol 0.25 mg/kg, nicardipine 15microgram/kg, n = 15). All patients received midazolam 0.5 mg/kg, and glycopyrrolate 0.2 mg IM for premedication. Fentanyl 1microgram/kg was injected before induction of anesthesia, and then esmolol, nicardipine, or the mixed drugs were administrated as an IV bolus and immediately followed by the induction drugs: thiopental 5 mg/kg, and succinylcholine 1 mg/kg. Endotracheal intubation was performed 90 seconds after injection of the experimental drugs. Thereafter 50% Nitrous Oxide in oxygen and 2 vol % enflurane were inhaled. BP and HR were recorded at the resting state, before fentanyl injection (base {T0}), after injection test drug (T1), after induction (T2), immediately after intubation (T3), 2 min (T4), 3 min (T5), 4 min (T6), and 5 min (T7) after intubation. RESULTS: There was a significant attenuation in SBP, MBP, DBP after tracheal intubation on the nicardipine and mixed groups compared to the esmolol group. HR was significantly lower in the esmolol and mixed groups than in the nicardipine group after tracheal intubation. CONCLUSIONS: Combined administration of esmolol (0.25 mg/kg) and nicardipine (15microgram/kg) was effective in attenuating an increase of BP and HR during tracheal intubation in hypertensive patients.

ABSTRACT

This study is designed to compare the effectiveness of single-bolus lidocaine, fentanyl and esmolol at the time of postintubation to blunt the hemodynamic intubation response in hypertensive patients. Sixty mildly hypertensive patients, scheduled for elective surgery, were randomly assigned in a double blind to receive a preintubation dose : in the firat group(Group A),20 patients received 1.5mg/kg lidocaine ; in the second group(Group B), another 20 patients received 3 ug/kg fentanyl; and in the third group(Group C), another 20 patients received 100mg esmolol. Within the above mentioned framework, this study obtained the following results : Systolic arterial pressure (SAP), heart rste (HR) and rate-pressure product (RPP) all significantly increased 1 minute after intubation. Fentanyl was more effective in protecting sgainst increase in SAP than lidocaine, which proves to be significant as shown in P<0.05. The statistical result, however, did not show the effectiveness of fentanyl signifieantly differed from that of esmolol in this respect. Esmolol preloading was significantly more effective in blunting hesrt rate than any other drugs. On the other hand, preloading of esmolol or fentanyl was significantly more effective in blunting rate-pressure product thsn lidocaine,which turned out to be P <0.05. Given these results, the present study concludes that esmolol preloading provides a reliable protection against increases in both heart rate and systolic srterial pressure accompanying laryngoscopy and intubation in hypertensive patients.
